Case Management

West Madison Senior Coalition case management services are free of charge to West Madison residents over age 60. Our case managers will help you to find, and connect to, resources for living independently in the community. They will be happy to visit your home to talk about your needs and our services.

For example, case managers can help you to look for:

♥ Housing

♥ Local transportation options

♥ Understand, apply for, and work with government and specialty programs that you may be eligible for: such as Medical Assistance, Energy Assistance, and Project Home

♥ A case manager can also assess your eligibility for our home chore volunteer services.

Prairie Park Senior Apartments, 6530 Schroeder Road:
First Friday of each month from 9:00 - 11:00 AM

Rosewood Villas, 902 N High Point Road
Second Friday of each month from 10:00 AM - 12:00 PM

Greentree Glen Senior Apartments, 732 Struck Street
Third Friday of each month from 1:00 - 3:00 PM

Lussier Community Education Center, 55 S. Gammon Road
Fourth Friday of each month from 11:30 AM - 1:30 PM.

Home Chore Help

Volunteers help with inside and outside cleaning and maintenance of your home or apartment. There is an income guideline for this program. Please contact a Case Manager for more information by calling 238-7368.

Home chores include: vacuuming, dusting, mopping and dish washing.
Outdoor chores include: mowing the lawn, raking leaves and shoveling sidewalks of seniors' homes.
