Please Note: Our Foot Care clinics will NOT trim nails for people with diabetes. However, Middleton Senior Center located at 7448 Hubbard Avenue, has a foot care clinic specifically for trimming toenails for people with diabetes on the 2nd Tuesday of each month from 1:00 - 2:45 PM. You must call for an appointment: 831-2373. Their cost is $25 All foot care clinic sites can be reached by elevator or have no steps. Olive Lutheran Church Our Lady Queen of Peace Church Farmer's Market Voucher Distribution on Madison's West Side:West Madison Senior Center, 602 Sawyer Terrace Starting in June, the Area Agency on Aging of Dane County, will distribute farmer's market vouchers to seniors for use in purchasing $25 worth of fruit, vegetables or herbs at Farmer's Markets and roadside farm stands in Wisconsin during the 2013 season (June 1st to October 30th, 2013). The vouchers are distributed on a first come/first serve basis to those 60 or older, who have gross incomes under the 180% poverty level. One set of vouchers per household regardless of size of household. Vouchers cannot be mailed. To get vouchers, you will be asked to prove your eligibility. Please bring along a picture ID showing your birth date, plus one of the following showing your income: 2011 or 2012 income tax, Homestead Credit form, social security allotment statement, or subsidized housing review letter. If you need someone to pick up vouchers for you, an application needs to be completed and signed by you and brought along when your designee picks up the vouchers for you, along with an ID and one of the above forms to prove your income eligibility. To receive an application prior to pick up dates, contact a case manager at the West Madison Senior Coalition at 238-7368 or Mary Browning at 261-5678. If you received vouchers last year, you will be sent a letter mid-May with all the distribution locations and times. No appointment is necessary. If you have vouchers, free transportation is available to the Wednesday Farmers' market at Hilldale Mall. Simply show a voucher to the bus driver. To make bus reservations, call Transit Solutions at 294-8747. Without a voucher, the cost is $2.00 roundtrip.
